Promus Diagnostics is the first PCR laboratory to report in true clinical units of CFU/mL. Most other PCR laboratories report culturable organisms in DNA copies/mL, a metric that is neither equivalent to CFU/mL nor consistent with current treatment guidelines. Reporting in DNA copies/mL can lead to overtreatment, directly opposing our commitment to antimicrobial stewardship.

For over 50 years, culture and sensitivity (C&S) testing has been considered the "gold standard" for identifying and diagnosing infections. This traditional method quantifies infectious burden using colony-forming units per volume (CFU/mL) and employs antibiotic susceptibility testing (AST) to determine antibiotic resistance and recommend therapy.

However, C&S has significant limitations. C&S often fails to grow or identify many pathogens that PCR technology can detect. This can result in undetected infections, compromising patient care. Additionally, C&S typically takes over 48 hours to complete, with an additional 24 hours needed for AST, totaling more than 72 hours. During this time, healthcare providers face the difficult decision of whether to treat empirically or delay treatment, potentially prolonging patient discomfort and increasing the risk of complications.

PCR is increasingly being adopted in the medical field. Currently, most PCR labs can identify 10-15 relevant pathogens per panel and detect 5-10 antimicrobial-resistant genes. Results are typically available within 48 hours of specimen arrival, which is both faster and more comprehensive than culture and sensitivity (C&S) testing.

However, many pathogens and resistance genes still go undetected, potentially leading to undertreatment or insufficient information for evidence-based decision-making. Additionally, many PCR labs report results in units of DNA per volume (copies/mL) rather than the more familiar CFU/mL used in current guidelines. This inconsistency can create challenges for medical providers when treating patients based on established CFU/mL standards.
